2. Include: (a) Type of training required, (childcare, health care, etc.)
(b) preferred date(s), location, and time,
(c) optional date(s), location, and time,
(d) your contact information.

3. Have a group size of at least 5 people*.
Per person / group rate is determined by type of class, and travel distance.

4. Sign a class quote / training confirmation form, and return the original copy to us.

5. Have a TV/VCR available (CPR or Safety Training). To host an onsite fitness
workshop and certification, space and availabe fitness equipment needs to be further
discussed.
